Privacy policy

This policy explains how Clear Health Medical LLC ("Clear Health Medical", "we") handles information collected through clearhealthfl.com. This site helps you find and contact one of our practices: Clear Ketamine Therapy, Clear Men's Health and Clear Medical Marijuana. It is not a patient portal, and it is not where medical care takes place.

What we collect

What you type into the request form: your name, phone number, email address, the practice you chose, whether you prefer an in-person or telehealth visit, the best time to reach you, and how you prefer to be contacted. For medical marijuana requests, we also ask whether you are a new patient or renewing.
How you found us: campaign tags in the link you followed (such as utm parameters) and the referring page, so we know which outreach is working.
Basic technical data: your browser type, plus a one-way hash of your IP address that we use to stop spam. We do not store your raw IP address alongside your request.

The form never asks for diagnoses, symptoms, medications, lab results or records. Please don't include them. Choosing an answer under "What brought you here?" only changes what the page shows you; that choice is not saved or sent anywhere unless you go on to submit the form.

How we use it

To pass your request to the practice you chose, or to help you choose if you weren't sure.
To contact you by phone, text or email about that request, using the method you asked for.
To keep the site secure and to understand, in aggregate, how visitors find us.

We do not sell your information, and we do not share it with advertisers.

Texts and calls

When you check the consent box, you agree that Clear Health Medical and the practice you chose may contact you by phone, text or email about your request. Message and data rates may apply. You can reply STOP to any text to opt out, or ask us to stop contacting you at any time. Consent is never a condition of receiving care.

Booking a visit

The booking page embeds Google Calendar appointment scheduling, which Google operates. If you reserve a time there, the name, contact details and appointment information you enter are collected by Google and shared with us so we can see the booking. Google's privacy policy applies to that scheduling page. We use what it sends us only to schedule and confirm your visit.

Who we share it with

Your request goes only to the Clear Health Medical practice it concerns, and to service providers who help us operate this site and deliver messages (for example, our hosting and email providers). Those providers may use the information only to perform those services for us. We may also disclose information when the law requires it.

Health information

Once you become a patient, any health information you share with a practice is protected health information, and is handled under that practice's privacy policy and Notice of Privacy Practices.

How long we keep it

We keep form requests only as long as we need them to follow up with you and to keep reasonable business records, after which they are deleted.

Your choices

You can ask us to update or delete the information you submitted through this site, or to stop contacting you. Call us at (656) 269-4567, Monday to Friday, 9am to 5pm.
