Notice of privacy practices

Your medical care happens at one of our practices, and each practice gives you its own Notice of Privacy Practices. That notice is the one that governs how your protected health information is used and disclosed. This page tells you where to find it and summarizes the rights it describes.

Your rights, in summary
Under federal law, as a patient you generally have the right to:
- See or get a copy of your medical record.
- Ask us to correct information you believe is wrong or incomplete.
- Ask us to contact you in a specific way or at a specific address, for confidentiality.
- Ask us to limit what we use or share, including with your health plan if you pay in full yourself.
- Get a list of certain disclosures we have made.
- Get a paper copy of the notice.
- Choose someone to act for you.
- File a complaint if you believe your rights were violated, without retaliation.
